VASER (Ultrasonic) Liposuction Technique:
There is a significant difference between the currently practiced liposuction techniques. The "traditional" suction-assisted or tumescent liposuction procedure uses a more "invasive" approach that includes general anesthesia, 10 to 15 mm length incisions, large bore cannulas, and a single arm to-and-fro movement for fat removal.
"VASER Liposuction" that Dr. Troell provides is safe and a minimally invasive procedure with less bruising, fast recovery time and with the superior advantage of creating significant skin tightening compared to all other modalities.
High-definition body sculpting can also be attained using the ultrasound technology. The worlds body contouring surgeons gathered in New York City at the World Congress of Liposuction to discuss techniques and scientific studies.
Dr. Troell was honored to be invited and presented three clinical studies he has completed. One study evaluated the quality of fat harvested using the VASER liposuction system with up t0 92% fat cell viability and 80% fat grafting survival. Initially, anesthetic fluid is infiltrated using small16 gauge blunt tipped cannulas. This is followed by the innovative technique of delivering ultrasound energy of higher and lower pressure waves at 36,000 Hz from an innovative titanium probe. VASER or Ultrasound-Assisted Liposuction gets its name by using gentle ultrasonic energy to selectively target fat cells while preserving important tissue, such as nerves, blood vessels and muscle.
Why Fat Grafting using VASER (Ultrasonic) fat is superior:
Fat Harvesting and Fat Grafting using VASER Fat is superior to that from laser lipolysis, such as Smartlipo, and suction-assisted liposuction. Fat collected during the VASER Lipo procedure is refined down to small lipocyte packets comprised of 2-3 fat cells, which supports growth and a blood supply (vascularization) upon placement in facial or body areas (reinjection). Laser lipolysis is inferior for fat grafting.
Lasers (SmartLipo, SlimLipo) deliver high intensity light energy and cause disruption of tissue, including fat cell rupture through thermal cell wall disruption and coagulation of small blood vessels which can burn the skin, creating cosmetic defects. Suction-assisted liposuction provides large cell packets, approximately 50 cells in diameter. This size is associated with central necrosis and sheer force disruption during fat transfer.
The fat used as a filler is permanent, in contrast to dermal fillers, such as Restylane and Juvaderm, which are temporary. In summary, the advantages of VASER Liposuction is less bruising and blood loss with a low revision rate, however, the biggest advantage is superior skin tightening and ability to create high-definition body sculpting. Additionally, VASER fat provides superior fat harvesting with a fat grafting survival rate of 80%. The fat grafting survival rate with traditional suction-assisted liposuction is, at best, 60%.
What is VASER® Lipo? How does it differ from traditional liposuction?
VASER Lipo, also known as LipoSelection®, is an advanced body contouring procedure that selectively removes unwanted body fat. An alternative to the harsh techniques of traditional liposuction, VASER Lipo uses state-of-the-art ultrasound technology designed to gently reshape your body. What distinguishes the VASER Lipo procedure is its ability to differentiate targeted fat from other important tissues – such as nerves, blood vessels and connective tissue. Innovative VASER technology breaks up fat while preserving these other important tissues to promote smooth results and rapid healing.
How does VASER Lipo work?
First, the area to be reshaped is filled with a special saline solution that helps numb the site and shrink the blood vessels (to minimize blood loss and reduce bruising). This solution wets and fills the area to be treated, making it easier to break up fatty tissue with the VASER Lipo System’s ultrasound energy. Then a small probe transmits sound energy (similar to that used for cataract removal from the eye) to break up fatty tissue on contact while preserving other important tissues. Finally, the liquefied fat is removed through a gentle suction process designed to minimize damage to surrounding tissues.
What body areas can be treated with VASER Lipo?
All body areas can be treated with liposuction. The most common areas are: abdomen, thighs, knees, buttocks, hips, back, arms, male and female breasts, love handles, chin, and neck.
How much fat can be taken?
There are limits to how much fatty tissue can be safely taken from the body, however, from a quantity standpoint it is 4 liters at one setting in California. One way to get a good idea for yourself as to the amount of fat to be removed, one can stand and bend from the waist infront of a mirror when evaluating the stomach and hips. The amount of ‘loose-hanging’ fat you can easily grab above / below your waist will approximate what can be removed. Bear in mind that this procedure is not intended as a weight loss solution. The goal is to create a slimmer silhouette. You may not notice a significant amount of weight loss, but you should be pleased with your slimmer new shape.
Where is VASER Lipo performed?
VASER Lipo is performed in a clean or sterile environment. Dr Troell has a surgical procedure room in our facility with complete cardiovascular monitoring, intravenous assess and two registered nurses available for assistance as well as medical assistants. Alternative locations include an outpatient surgery center or a hospital. The procedure can be performed under oral sedation with local anesthesia (awake), IV sedation, or general anesthesia.
How soon will I see my results?
This varies with the extensiveness of the procedure—the amount of fat removed, the number of sites treated, and so on. Many patients report that they see results immediately after the procedure with the final result at around 3 to 6 months. It is important that you establish realistic expectations through discussion with your doctor.
What is the recovery time?
Only one patient in the past ten years experienced discomfort after three post-operative days requiring narcotic pain medications. The muscles are sore for about 7-14 days, however, only nonsteroidal anti-inflammatory medications (Alleve, Motrin, Ibuprofen) are needed. The incisions drain fluid for 18-24 hours. A compression garment is recommended for three weeks, instead of 6 weeks like some surgeons recommend. Dr Troell performed a study revealing no change in outcome with less duration of compression. The main risk of a complication after liposuction is not getting a smooth outcome. It is more likely to achieve a smooth outcome, because the ultrasound "melts" the fat and you require less liposuction time during the procedure. The only other complication of any frequency is a small fluid collection of triglycerides and salts from dead fat cells that do not leak out of the incisions or are not absorbed by the body.
What about future weight gain / fat migrating to other areas?
Once fat is removed it is gone forever. A Brazilian study revealed if a man had abdominal and hip liposuction and weighed the same after surgery, that the fat cells that became more engorged with triglycerides was inside the abdominal cavity. This visceral fat is present in the omentum, which harbors the blood vessels and acts as a blanket to protect the intestines and organs from blunt trauma. Following your VASER Lipo procedure, most patients who experience weight gain after liposuction, will observe the fat to be proportionately distributed over your entire body. Once you have fat removed, it is gone and, assuming a healthy lifestyle of proper diet and exercise, it will not return or migrate. However, because fat is necessary to a healthy body, some essential fat layers in any given site remain untreated.
