Steps in Cosmetic Eyelid Surgery:
- Exam that notes the eyelid and face concerns
- Eye visual acuity testing and testing tear production if dryness is present, screen for thyroid disease

Forehead Lift/Brow Lift- these procedures may be needed to optimize the appearance of one's eyes if the brow is drooping along with excess upper eyelid skin. If this is the case, the brow lift procedure is performed first, followed by the eyelid surgery, often at the same surgical setting. Dr Troell has written a peer reviewed article to assist plastic surgeons choose the optimal forehead lift procedure while at Stanford University Medical Center.
Mid-Facelift- this procedure may be needed to optimize the appearance of the lower eyelids and cheeks. When the midface droops or is pulled downward by the aging process, rejuvenation of the eyelids in concert with the midface is the ideal aesthetic decision. The lower eyelid may be pulled down enough to see the white sclera below the colored part of the eye, known as the iris, which dramatically changes the beauty of the eye.
Laser Skin Resurfacing- the laser procedure can be performed at the same time of upper and lower eyelid surgery to improve the two most significant aspexts of our face, the eyes and our skin. Canthopexy- Strengthening the lateral aspect of the lower eyelid can improve those with excess tearing (epiphora) or dry eye symptoms. when the lateral aspect of the lower eyelid is pulled downward due to the aging process (senile ectropion or scleral show), from previous eyelid surgery that removed too much skin or from the midface pulling the eyelid inferiorly, strengthening the lateral canthus by re-suspending this aesthetically important structure further rejuvenates the face.
Dermal fillers or autologous fat transfer- Often there is a deficiency of volume in the tear trough, which is the are immediately below the lower eyelid, which looks more rejuvenated when filled. A dermal filler such as Juvaderm, Restylane or Bellafill (Artefill) can be placed here, best using a blunt tip cannula such as Dermasculpt. Alternatively, patients can have fat harvested from their body and injected into the tear trough, midface area and lower face areas for optimal rejuvenation of the volume lost by the aging process.
